Roaccutane (Isotretinoin)

Isotretinoin (Roaccutane), a chemical cousin of vitamin A, is prescribed for the treatment of severe, disfiguring cystic acne that has not cleared up in response to milder medications such as antibiotics. Accutane works on the oil glands within the skin, shrinking them and diminishing their output. Take Isotretinoin by mouth every day for several months, then stop. The anti-acne effect can last even after you have finished your course of medication.

Severe nodular acne is when many red, swollen, tender lumps form in the skin. These can be the size of pencil erasers or larger. If untreated, nodular acne can lead to permanent scars.

Roaccutane Dosage

The recommended dosage range for Isotretinoin is 0.5 to 1.0 mg/kg/day given in two divided doses with food for 15 to 20 weeks. In studies comparing 0.1, 0.5, and 1.0 mg/kg/day,8 it was found that all dosages provided initial clearing of disease, but there was a greater need for retreatment with the lower dosages. During treatment, the Isotretinoin dose may be adjusted according to response of the disease and/or the appearance of clinical side effects — some of which may be dose related. Adult patients whose disease is very severe with scarring or is primarily manifested on the trunk may require dose adjustments up to 2.0 mg/kg/day, as tolerated. Failure to take Roaccutane with food will significantly decrease absorption. Before upward dose adjustments are made, the patients should be questioned about their compliance with food instructions.

After you finish taking Roaccutane, there should be at least a 2-month "rest period" during which you are off the drug. This is because your acne may continue to get better even though you are no longer taking the medication. Once the 2 months are up, if your acne is still severe, your doctor may want to give you a second course of Isotretinoin. If you are still growing, your doctor may recommend a longer "rest period."

If you miss a dose of Roaccutane

Take the forgotten dose as soon as you remember. If it is almost time for your next dose, skip the one you missed and go back to your regular schedule. Do not take 2 doses at the same time.

Storage — How to store Roaccutane

  • Keep Roaccutane out of the reach of children.
  • Store Roaccutane away from heat and direct light.
  • Do not keep outdated Roaccutane or medicine no longer needed. Be sure that any discarded Isotretinoin is out of the reach of children.

Side effects of Roaccutane

Side effects can not be anticipated. If any develop or change in intensity, inform your doctor as soon as possible. Only your doctor can determine if it is safe for you to continue taking Accutane.

Because Roaccutane can cause severe birth defects, including mental retardation and physical malformations, a woman must not become pregnant while taking it. If you are a woman of childbearing age, your doctor will ask you to sign a detailed consent form before you start taking Roaccutane. If you accidentally become pregnant while taking the medication, you should immediately consult your doctor.
